Perioperative considerations in the management of a patient with coronary artery disease undergoing noncardiac surgery

چکیده
1. If the surgery is elective or less urgent and the cardiac disease is of such a magnitude that surgical correction is indicated, then it is preferable to perform the cardiac surgery [or some relevant interventional procedure such as percutaneous transluminal coronary angioplasty (PTCA) for coronary artery disease (CAD) or balloon mitral vlvotomy (BMV) for mitral stenosis (MS)] before noncardiac surgery is attempted. Alternately, if the cardiac disease does not warrant surgical correction, then the medical management should be optimised before subjecting the patient to noncardiac surgery. 2. If the surgery is of emergent nature and does not allow sufficient time for the treatment of cardiac lesion, then surgery should be undertaken with an elevated or low risk. This stratification should be decided after preoperative evaluation. However, an attempt should be made to optimize the cardiac condition by medical management (such as diuretics, inotropes, dilators) or mechanical ABSTRACT

Noncardiac surgery following percutaneous coronary intervention.
Patient with coronary artery disease (CAD) undergoing major noncardiac surgery (NCS) are at increased risk of serious perioperative cardiac complications. At the same time, safety of percutaneous coronary intervention (PCI) before noncardiac surgery has been questioned. This paper reviews the available literature regarding the safety of PCI before NCS. At the same time, cardiac evaluation befor...
متن کاملThe effects of prophylactic coronary revascularization or medical management on patient outcomes after noncardiac surgery--a meta-analysis.
PURPOSE The benefits of prophylactic coronary revascularization for patients undergoing noncardiac surgery are uncertain. The purpose of this study was to systematically evaluate the effect of coronary revascularization and medical management on short- and long-term outcomes after noncardiac surgery. METHOD Ten electronic databases including MEDLINE and EMBASE (1980 to February 2006), and bib...
متن کاملPreoperative Assessment of the Patient with Cardiac Disease
The past several years has seen a dramatic increase in the number and quality of randomized and prospective studies to define the optimal and most cost-effective approach to preoperative cardiovascular evaluation and management for noncardiac surgery, including studies evaluating the role of coronary revascularization before noncardiac surgery and perioperative beta-blockers. In 2007, the Guide...
متن کاملPerspectives on the management of antiplatelet therapy in patients with coronary artery disease requiring cardiac and noncardiac surgery.
PURPOSE OF REVIEW To provide an updated overview on the management of antiplatelet therapy in patients with coronary stents undergoing cardiac and noncardiac surgery. RECENT FINDINGS Surgical procedures are frequently performed in patients with coronary stents and are associated with an increased risk of ischemic and bleeding complications in the perioperative period. Given the lack of well-d...
